The Calcium Problem

In 2003, at age 18, Will Johnson had a seizure. The cause: severe hypocalcemia — dangerously low serum calcium — from a condition called hypoparathyroidism. The parathyroid glands had failed. PTH (parathyroid hormone) was absent. Without PTH, the body cannot regulate calcium, and calcium is load-bearing for almost every biological system: nerve conduction, muscle contraction, cardiac rhythm, and — it turns out — cognition.

For 22 years, the management was blunt: high-dose calcium and vitamin D supplements taken multiple times daily. Serum calcium levels would drift. On bad days, cognitive performance degraded visibly — slower processing, working memory gaps, difficulty converting internal state into speech. The symptoms were real. The mechanism wasn't fully understood. Clinicians had few tools beyond "take more calcium."

Then in 2024, the FDA approved Yorvipath (palopegteriparatide) — the first prodrug of PTH(1-34) for chronic hypoparathyroidism in adults. The PaTHway trial showed 95% of patients achieved independence from conventional calcium/vitamin D therapy. Annual cost: $150,000–$285,000. For Will, it remains inaccessible.

Also in 2025, a prospective study published in the Journal of Clinical Endocrinology & Metabolism confirmed what Will had known from the inside for two decades: 56.7% of hypoparathyroidism patients show measurable attention and processing speed deficits (p<0.001), and 60% show executive function deficits. Processing speed correlates directly with serum calcium levels (r=0.53, p=0.023). The cognitive load is not imagined — it is physiologically grounded and now measurable.

The Posner Molecule and the Name

The Posner molecule is Ca₉(PO₄)₆ — a calcium phosphate cluster found in biological systems. It's a calcium cage that protects a core of phosphate groups, including phosphorus-31 (31P), the only stable isotope of phosphorus.

Phosphorus-31 is the element that gives P31 Labs its name. In bone mineral and in Posner clusters, calcium and phosphorus are structurally coupled — you cannot have one without the other. In hypoparathyroidism, the calcium regulation system fails, and the phosphate system dysregulates with it. The body cannot maintain the geometry it needs.

The metaphor extends: P31 Labs is the calcium cage. The tools we build — BONDING, The Buffer, Spaceship Earth — are the structure that protects what matters inside. The organization's entire architecture is designed to hold together when the external system fails.

Note: The S₆ symmetry of Posner clusters at physiological temperature is currently contested in the literature (Agarwal et al., JPCL 2021). The structural coupling between calcium and phosphorus chemistry is not.
